West Toronto Ontario Health Team

The West Toronto Ontario Health Team (WTOHT) is a collaborative network of 38 health and social service providers in West Toronto, Ontario. Established in November 2020, WTOHT aims to enhance the well-being and health outcomes of approximately 527,000 residents by creating a connected system of care. The organization focuses on integrating services from primary care, hospitals, home care, and community resources to provide coordinated support for patients, families, and caregivers.

WTOHT offers various services and tools designed to improve access to care and address social determinants of health. Key initiatives include Primary Care Matching to help residents find healthcare providers, a Mental Health & Addiction Asset Map featuring over 70 services, and community resources linking to health and social service directories. The team emphasizes equitable access and continuous learning, engaging community members and partners in its planning and operations. Through digital tools and navigational resources, WTOHT enhances the experience of healthcare for all demographics in the region.
